Assessment forms and calculated totals
Review assessment questions and configured arithmetic, test expected totals, and manage the resulting data without treating scores as diagnoses.

An assessment form can collect answers and calculate a configured total so staff do not have to add the values by hand. The useful result is a traceable set of responses and the arithmetic configured for that form. Review the questions and formula before using the result in your workflow.
Start with the intended instrument
Choose a template or create a form for the assessment your organization has selected. Compare the actual questions with the instrument source: item wording, response options, recall period, requiredness and respondent instructions can affect what the answers mean.
A template title does not establish that its current form contains every item, that a total has already been configured, or that you have the permissions needed to use the instrument. Keep that review separate from the technical setup.
Check the calculated field
- Open the assessment in the editor.
- Identify the field that stores the total and inspect the configured answer values or formula.
- Check missing-answer behavior, reversed items and any subscale calculations required by your chosen specification.
- Use synthetic answer sets whose expected totals you have calculated independently.
- Submit the examples and compare the stored totals in the response and output report.
Use a low, middle and high example plus any boundary case relevant to the formula. If an unanswered item should prevent scoring, check that case explicitly instead of assuming blanks always count as zero.
Keep the output useful
Use a clear total label, and keep the underlying answers available to authorized staff. Configure reports or Sheets output to contain the columns needed for the task. If the form collects health information, review PHI controls before exporting or notifying staff.
A calculated total is not automatically a clinical conclusion. Formesign performs the configured calculation; your organization's chosen instrument guidance and qualified staff determine how the result should be used. Avoid adding a diagnostic statement merely because a number crosses a threshold.
Review changes before collecting again
Recheck the score after changing a question, option value, required field, copied template or formula. Keep a record of the configuration used for the collection period so comparisons are meaningful.
If a total is missing, first confirm the field is configured and that the necessary answers were submitted. Then inspect whether the chosen report includes the calculated column or omits a marked sensitive field. For patient intake that uses an assessment as one component, link the assessment and intake process through the actual staff workflow.
