Google Forms and response Sheets
Understand the separate source, form-response and report-to-Sheets connections.

Choose the destination your team intends to read before enabling synchronization. A spreadsheet used as the source of a Sheet-signature request, a Sheet linked to Google Forms, and a Sheet connected to a report are different relationships.
Identify the data path
| Path | What to check |
|---|---|
| Formesign response → Google Forms → linked response Sheet | Response forwarding must be enabled for that form, and the underlying Google Form must be linked to the intended Sheet. |
| Formesign report → report Sheet | Configure the report's Sheets connection and verify its selected columns and destination. |
| Source spreadsheet → Sheet-signature request | This starts a signing request from spreadsheet content; it is not the same as appending response rows. |
Use a Google Forms-linked Sheet
Confirm the underlying Google Form associated with the current Formesign form. Where the form settings expose Submit to Google Forms, check that forwarding is enabled. Open the appropriate Google Form's Responses area and link the desired response spreadsheet using Google's controls.
If you imported a copy, verify which Google Form is receiving responses. The original form's old response Sheet may not be the destination of your newly created version.
Connect a report
Open the intended report and use Link to Sheets or the Sheets export control shown there. Choose the account and destination offered by the flow, then review the selected columns and PHI decision. Do not assume this connection has the same update behavior or field permissions as Google Forms forwarding.
Check the result
Use a synthetic response with a recognizable identifier. Find it first in Formesign, then check the expected Google destination and columns. Confirm that any PDF link opens the intended record for an appropriately authorized viewer.
For sensitive data, inspect the exact route: PHI handling in report exports does not prove that the external Google Forms-linked Sheet has identical restrictions. Review the Sheet's Google sharing permissions too.
If a row is missing, trace this route in order using record and sync troubleshooting. To start a request from a spreadsheet instead, use Sheet signature.
