Show terms, consent and a response summary

Let respondents review their answers and the terms before submitting.

Reviewed 2026-10-30 · Formesign guides

Put the information a respondent needs to review before the final signing or submission action. Depending on your setup, this can be document text, a consent screen, or a summary of the answers they are about to submit.

Choose the review task

A response summary lets people check their answers. Terms or consent text explains what they are acknowledging. A signature records a signing action. Configure each task for its purpose so the respondent can tell what they are completing.

The existing Document signature resources include Show consent screen, Publish as PDF and review-before-submit topics. These resources cover different add-on and document setups; use the controls available in your current surface rather than expecting every older menu in the web editor.

Prepare the content

  1. Put the exact wording to be reviewed in the intended document or consent content.
  2. Check that respondent-specific values are mapped to the right fields and displayed beside that wording where appropriate.
  3. Configure the review or consent control available for the form. If your add-on exposes Show consent screen, preview its effect before distributing the form.
  4. Check any Agree, signature and submit actions in order so the user understands the final step.
  5. Inspect the resulting response and PDF to see what is retained.
Review the document content and the final action together.

Check the respondent experience

Open the actual shared link on desktop and mobile. Make sure the document is readable, the summary contains the expected answers, and a signer can return to correct an answer when that flow permits it. Test unanswered required fields so they cannot accidentally bypass an essential question.

If you turn a consent screen off, preview again and check that required information still appears before signing. A change to the review screen can change what future respondents see even when the form's sharing link stays the same.

Keep the record appropriate

Use wording supplied or reviewed for your organization's purpose. An Agree button or signature field does not by itself establish that a particular legal, clinical or regulatory requirement has been met. Describe the action accurately and retain the resulting record according to your process.

For document mapping, see PDF fields. For the completed copy, see signed PDFs. If a summary omits a value, check both its question and any field privacy restrictions before changing the review content.
