Quick Answer
How do I show a consent screen before submitting?
In the EDIT tab, find the After page card at the end of your form and click its gear. Open the On submit tab and check Show consent screen while submitting form. Write the wording in the Consent tab, then click Save.
There are two ways to show people what they're agreeing to:
| Method | What people see | Best for |
|---|---|---|
| Terms on the form | Wording in a section, with an I agree question | Terms people should read as they go |
| Consent screen | A step that appears when they submit | A final confirmation |
Many forms use both. A generated handbook acknowledgement, for example, puts the wording in a section followed by a required I agree checkbox.
Add terms to the form
- In the EDIT tab, add a section above the signature, using the Tт button on the right-hand toolbar.
- Put the wording in the section's description.
- Add a checkbox question such as I have read and agree to these terms, and mark it required.
Then move the signature after it. Long wording shows a Show more link in the editor, and appears in full to signers and in the signed PDF.
Turn on the consent screen
Open the submit settings
In the EDIT tab, scroll to the After page card at the end of the form and click the gear.
Show the consent screen
Open the On submit tab and check Show consent screen while submitting form.
Write the wording
Open the Consent tab, enter the wording, and click Save.
Test it
Open the signing link and submit a practice response. To turn the screen off later, uncheck the box and test again.
What happens after submit
The same card controls what signers see afterwards.
| Choice | What signers see |
|---|---|
| Default message | Your response has been recorded. |
| Custom message | Your own confirmation |
| Redirect to webpage | A page you choose |
If someone else still has to sign, say so. "Your request has gone to your manager for signature" is clearer than "Thanks, you're all done".
The Button tab sets the form's Language and the submit button's Caption.
Consent is your wording
Formesign records what was shown and who signed. Whether that wording meets a legal or clinical requirement is for your organization to decide, so have it reviewed.
